This systematic review aimed to compare the effects of consuming equal amounts of fat from cheese and other dairy products on blood lipid markers in both fasting and postprandial states. The authors searched PubMed (Medline), Cochrane Central, and Embase databases up to mid-June 2022 for eligible randomized controlled trials (RCTs) that investigated the effect of isoenergetic substitution of hard or semi-hard cheese with other dairy products on blood lipid markers.
The meta-analysis of 7 RCTs showed that consuming an average of 135 g of cheese daily for ≥14 days led to a reduction in fasting total cholesterol, LDL-C, and HDL-C concentrations compared to consuming ∼52 g/d of butter. However, there was no evidence of a benefit from replacing cheese with milk for ≥14 days on fasting blood lipid markers (n = 2).
Limited postprandial RCTs suggested that cheese-rich meals may induce different fed-state lipid responses compared to some other dairy matrix structures, but not butter (n ≤ 2). The authors concluded that dairy fat consumed in the form of cheese has a differential effect on blood lipid responses relative to some other dairy food structures. However, due to considerable heterogeneity and limited studies, further confirmation from RCTs is needed.

In this randomized crossover trial, daily consumption of low-fat yogurt plus butter increased the levels of a cholesterol-carrying protein linked to heart disease, compared to full-fat yogurt, despite the 2 yogurts containing the same amount of dairy fat.
